Low Dose Naltrexone (LDN)

Low Dose Naltrexone (LDN) refers to the use of naltrexone at substantially lower doses than those traditionally used for opioid or alcohol dependence. Doses typically range from 0.5 mg to 4.5 mg daily. Naltrexone was initially approved for addiction medicine at much higher doses, but low doses are utilized for other clinical considerations.

What is LDN Used For?

LDN has been investigated for its effects on various chronic conditions such as:

  • Fibromyalgia
  • Chronic pain syndromes, including neuropathic pain and complex regional pain syndrome (CRPS)
  • Some immune-mediated conditions, including Crohn’s disease and multiple sclerosis
  • Post-viral fatigue syndromes

LDN is considered an off-label medication for these uses.

How Does LDN Work?

LDN is thought to exert its effect through:

  • Temporary modulation of opioid receptors, which may promote the body’s natural endorphin production
  • Impact on immune pathways, specifically by modulating microglial and Toll-like receptor activity, potentially affecting inflammation

Dosing Information

A typical starting dose is 0.5 mg to 1.5 mg daily, gradually increased to an optimal dose, often up to 4.5 mg daily, depending on patient response and tolerability. Dose titration protocols may differ per individual.

Tolerability and Potential Effects

LDN is generally well-tolerated. Some individuals may experience side effects such as vivid dreams, sleep disturbances, or gastrointestinal symptoms. These effects are usually mild and may lessen over time. LDN should not be used with opioid pain medications, as it can diminish their effect.
